Palliative Care
Palliative care is a continuum of care that can span from the point of diagnosis with a serious illness to death. Care can be provided in coordination with disease-directed care, such as chemotherapy and radiation for the patient with cancer for whom cure is the primary focus or for a patient with a poor prognosis who needs supportive care at end of life. In fact, there is a growing body of evidence that indicates that many patients benefit from a combination of both palliative care and curative treatment in cancer care, even prolonging life in some instances.
Surgical Recovery
Following an injury, illness or surgery, continuous rehabilitative care is necessary for a time to ensure as complete a recovery as possible for each patient. We specialize in overseeing your recovery with uninterrupted care for maximized rehabilitation outcomes and a higher quality of life. The average inpatient length of in-home care is three to five days/nights, depending on the patient’s rehabilitation needs and progress.
Medical Management
We believe that caring for a condition can be more impactful than caring for a symptom. Partnering with a patient’s healthcare team, our disease-specific programs can help impact patient outcomes, reduce re-hospitalizations and enhance the overall life of the patient.
